On Call Nurse Hospice

Deliver direct patient care and coordinate with physicians, social workers, and home health aides in hospice patients' homes. Meet discipline-specific productivity standards and demonstrate clinical competency through field observation by supervisors. Complete and submit documentation following established guidelines while communicating progress toward goals and physician orders. Ensure compliance with agency, state, and federal regulations including U.R reviews and patient care planning. Maintain a California driver's license and auto insurance to respond to after-hours patient needs and manage admissions when the interdisciplinary team is off.
