Job Summary
Perform after-hours and weekend visits, admissions, and patient follow-ups while coordinating prescription refills, supplies, and DME orders. Dispatch field nurses to patient residences and document all calls, conversations, and outcomes within the call-in software system. Provide telephone triage by directing inquiries to supervisors or managing needs immediately per scope of practice, while receiving and giving shift reports. Maintain comprehensive knowledge of information systems and travel throughout the Bristol Hospice Service Area to support admissions, discharges, and exceptional care for patients and families.
Required Qualifications
- LVN/LPN
- current licensure in good standing with the state
- knowledge of hospice
- understand the philosophy of hospice concept
- understand needs of the terminally ill
- flexible in work hours
- aptitude of computer data entry
- ability to deal with stressful situations
- ability to deal with grieving families
- nursing skills per competency checklist
- willingness to maintain comprehensive working knowledge regarding information systems
- working knowledge regarding applicable software programs
- ability to travel throughout the Bristol Hospice Service Area
- ability to lift heavy items on a frequent basis
- ability to lift and carry up to 50 lbs.
- ability to spend a prolonged amount of time standing
- ability to spend a prolonged amount of time walking
- ability to spend a prolonged amount of time bending
- ability to spend a prolonged amount of time stretching
