On-Call Hospice LPN

Job Summary
Administer medications and treatments per hospice policies, perform baseline nursing skills, and monitor patient conditions while reporting changes to the supervising RN. Develop and update patient-centered Plans of Care, maintain accurate documentation, and provide emotional support to patients and families. Communicate effectively with physicians and interdisciplinary team members, participate in team conferences, and promote teamwork. Maintain patient confidentiality, adhere to HIPAA policies, and complete required continuing education. Requires current LPN license in New Mexico, CPR certification, and at least one year of nursing experience. Occasional on-call rotation may be required.
Required Qualifications
- Graduate of an accredited Licensed Vocational/Practical Nursing Program
- Current and valid LPN license in the State of New Mexico
- CPR certification
- Valid driver's license with auto insurance
- At least 1 year of nursing experience
- Strong organizational, time management, and communication skills
- A cooperative, compassionate approach to patient care
- Ability to handle emotional demands with grace and professionalism
- Basic math skills for medication and treatment administration
- Maintains patient confidentiality and adheres to HIPAA and privacy policies
- Participates in continuing education and in-service trainings
- Keeps current with health care regulations and standards
- May be required to participate in occasional on-call rotation
