Job Summary
Conduct on-call and after-hours nursing care for hospice and home health patients, responding to triage calls and completing scheduled visits outside business hours. Assess patient status, revise care plans in collaboration with the RN and medical director, administer prescribed medications, and provide health care instructions to patients and caregivers. Document services accurately, coordinate with the interdisciplinary team, and ensure availability of necessary equipment and supplies. Requires current Oregon LPN licensure, CPR certification, and a licensed driver's license.
Required Qualifications
- Graduate of an accredited school of nursing
- Currently licensed as a Licensed Practical Nurse in the State of Oregon
- Possesses and maintains CPR certification
- Communicate effectively, both written and orally, with all levels of the organization
- Excellent observation, verbal and written communication skills
- Problem solving skills
- Basic math skills
- Nursing skills per competency checklist
- Self-directed and able to work with minimal supervision
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements
- Pre-employment criminal background check
- Drug screening
Desired Qualifications
- 1 - 2 years of recent acute care experience in an institutional setting
- 1 -2 years of recent experience in hospice or home health
