Job Summary
OMH-Resident Wellness Coordinator provides counseling, advocacy and case management services to residents in NYSOMH-funded Residential Programs, ensuring adherence to NYSOMH regulations and agency guidelines. Responsibilities include delivering counseling and behavioral interventions, developing wellness goals, making referrals to community services, documenting progress, participating in supervision and staff meetings, and accompanying clients to appointments. The role requires meeting residents regularly, coordinating services holistically, and traveling across the five boroughs. Bachelor’s degree in a related health/human services field is required; bilingual abilities are preferred; hourly pay with competitive benefits.
Required Qualifications
- Bachelor’s degree in social work, psychology or a related health/human services field required.
- Excellent clinical skills with at least two years relevant mental health, substance abuse or human service experience.
- Bi-Lingual Chinese/Korean/Spanish/English Preferred.
- Regularly required to talk, hear, walk, stand, & sit and lift up 10 lbs.
- Able to travel throughout the five boroughs as needed.
