Job Summary
Coordinate and lead the Wraparound Team; coordinate and supervise implementation of the Plan of Care, including Transition and Crisis Plans, through service delivery with providers and community resources; ensure family support and stabilization during crises; provide and document the Life Domain Assessments; maintain service documentation and data; utilize and monitor Flexible Funding and service coordination; obtain weekly reports from subcontracted providers; participate in after-hours on-call response; engage in program staff meetings, supervision, and CQI processes; meet agency training requirements; report MUIs; drive as required; role requires a Bachelor's degree with 2+ years of direct experience or 5+ years of related experience; located in Cincinnati, OH.
Required Qualifications
- Bachelors degree in related field and two years of direct experience or five years of related experience
- Must complete High Fidelity Wraparound model training within 12 months of employment.
