Job Summary
Recruit, interview, and train new Care Coordinators while leading a team to achieve monthly and yearly program goals. Conduct performance evaluations, develop corrective action plans, and provide regular supervision with verbal and written feedback to ensure open communication. Facilitate bi-weekly staff meetings and departmental gatherings to maintain quality, holistic treatment of families and consistency in program planning. Review program paperwork for documentation accuracy and treatment planning, oversee the development and implementation of all service plans, and ensure staff utilize approved strategies for clients and families. Guarantee adequate staff and crisis coverage during absences, develop appropriate aftercare plans, and ensure staff are trained in developmental strategies and working with children experiencing trauma, co-occurring disorders, and engagement with child-serving systems.
Required Qualifications
- Will be a licensed or an unlicensed practitioner in accordance with rule 5160-27-01 of the Administrative Code, except that an Intensive Care Coordinators or Moderate Care Coordinators, care coordinator supervisor will be employed by or under contract with a Care Management Entity as described in this rule.
- Have experience providing community-based services to children and youth and their families or caregivers in areas of children's behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field for: o three years with a high school diploma or equivalent; or o two years with an associate's degree or bachelor's degree; or o one year with a Master's degree or higher.
- Have a background and experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
- A supervisor that is an unlicensed practitioner will have regular supervision with a licensed practitioner and real-time access to a psychiatrist for case consultation.
- Supervisors of Intensive Care Coordinators or Moderate Care Coordinators will complete the high-fidelity wraparound training program provided by an independent validation entity recognized by Ohio Department of Medicaid.
- Have the qualifications and experience needed to work with children and families who are experiencing SED, trauma, co occurring behavioral health disorders and who are engaged with one or more child- serving systems. (e.g., child welfare, juvenile justice, education)
- Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs
- Be culturally competent or responsive with training and experience necessary to manage complex cases
- Live in one of the counties included in Catchment Area C
- Valid driver's license
- Reliable personal transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
