Job Summary
Conduct home-based, empirically based therapy for youth and their families, including completing diagnostic evaluations and developing treatment plans based on strengths and needs. Deliver activity and participation-based interventions, make referrals to community resources, and provide aftercare plans for each individual served. Maintain confidential, accurate documentation and participate in peer and collateral team meetings. Support professional development by addressing team performance concerns and guide families in interacting with referring agencies.
Required Qualifications
- LPC, MFT or LSW
- Two years related field experience
- Specialized training
- Exceptional communication skills
- Ability to establish trust and rapport quickly
- Good listener
- Ability to empathize
- Ability to provide guidance
