Office Nurse Practitioner

Office Nurse Practitioner role within Lexington Health network. Responsibilities include assessing patients’ physical and psychosocial status via interviews, health history, physical exams, and diagnostic tests; interpreting data and developing treatment plans; following through on continuum of care across health-illness lifespan within protocols approved by supervising physicians. Performs diagnosis and treatment of acute illnesses, prevention and maintenance for patients with stable chronic illnesses, and interim exams for healthy patients. Maintains accurate records and emphasizes health education, disease prevention, and appropriate referrals. Supervises and directs clinical staff as needed, participates in quality assurance, risk management, and ongoing professional education; prescribes within the practice agreement and adheres to departmental protocols. Ensures confidentiality and documentation accuracy, with emphasis on effective patient care and operational efficiency.
