Job Summary
Office-Based Home Health RN Case Manager responsible for supervising clinical staff, reviewing and approving patient assessments and care plans, monitoring quality assurance and performance improvement activities, assisting with staff training and scheduling, ensuring survey readiness and audit preparation, addressing clinical concerns, and coordinating patient care with physicians and interdisciplinary teams. Requires an accredited nursing program graduate (Diploma/ADN/BSN) and an active RN license; minimum 2 years clinical nursing and 1 year home health experience; driver’s license and CPR; benefits include PTO, holidays, health/dental/vision, and retirement options.
Required Qualifications
- Graduate of an accredited nursing program (Diploma, ADN, or BSN)
- Active, unencumbered RN license (state or compact/NLC license)
- Minimum 2 years of clinical nursing experience
- 1 year of home health experience
- Management experience preferred
- Current driver’s license
- CPR certification
