Job Summary
Provide longitudinal and urgent care to patients in an office location, developing adult and geriatric panels while educating patients on wellness visit benefits. Create individualized care plans, coordinate with nursing and support staff, and utilize Epic EHR to update charts and document self-management strategies. Diagnose medical conditions, prescribe pharmaceuticals and treatment regimens, and analyze patient history to determine best care. Float between locations in West Haven, New Haven, Milford, Guilford, and Wallingford as needed. Requires a master's or doctorate-prepared APRN or PA with two years of experience, valid Connecticut licenses, and DEA certification.
Required Qualifications
- Proof of annual flu vaccination
- Masters or doctorate prepared
- Certified Nationally as a Nurse Practitioner through AANP or ANCC
- Minimum of 2 years experience as a APRN
- Possession of a valid (Connecticut) license as a Registered Nurse
- Possession of a valid (Connecticut) license as an Advanced Practice Registered Nurse
- Certification as a Nurse Practitioner by an approved certifying body
- DEA license
- Ct controlled substance license
- Experience working with EHR
- Physician Assistant Certification
- State License
- Minimum of 2 years experience as a PA
- Must be available to float between locations in West Haven, New Haven, Milford, Guilford and Wallingford
