Job Summary
Office and patient coordinator provides administrative and clerical support for program staff, assists Senior Life Solutions patients with care, and coordinates transportation for patients using a provided vehicle. Responsibilities include insurance verification and billing procedures, clerical support to team members, assisting with patient care, completing census and medical records documentation, coordinating meals and snacks, maintaining refrigerator logs, and ensuring compliance with regulations. Duties involve taking patient vital signs, assisting with patient admissions and consents, participating in treatment team meetings, and contributing to a positive team and community presence. Requires high school diploma or GED; CNA, MA, or LPN certification preferred; experience with care for older adults and with electronic medical records; valid driving record and ability to transport patients."
