Ocupational Therapist

Occupational Therapist provides intermittent OT services in patients' residences under physician orders and plan of care, performing initial assessments, maintaining/adjusting the plan of care, and reassessments every 30 days. Collaborates with the care team to improve patient function and return to productive living within the patient’s capability. Requires OT licensure (or eligibility for NCE) and current state licensure; minimum one year in an acute care setting with two years in Home Health preferred.
