Job Summary
Conduct comprehensive palliative care assessments and follow-up visits for patients with serious illnesses and complex chronic conditions. Manage symptom management, facilitate goals-of-care conversations, and develop individualized care plans while collaborating with physicians, specialists, and community providers. Provide patient and family education, document encounters accurately, and participate in interdisciplinary team meetings and care coordination activities. Work within a collaborative interdisciplinary team to make a meaningful impact on patients and families through comprehensive symptom management and advance care planning.
Required Qualifications
- Active New Jersey Nurse Practitioner license
- National board certification
- DEA registration
- Minimum 2 years of Nurse Practitioner experience
- Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management
- Strong communication and interpersonal skills
- Ability to work independently while collaborating within a multidisciplinary team
Desired Qualifications
- Hospice & Palliative Care certification (ACHPN)
- Experience with home-based care, telehealth, or value-based care models
- Bilingual skills
