Job Summary
Provide Occupational Therapy services for patients in Assisted Living, Independent Living, and Memory Care settings under the direction of a licensed OT. Implement treatment plans for assigned patients, assist in initial screenings, and supervise Rehab Techs during group and individual sessions. Manage case responsibilities, contribute to interdisciplinary team activities, and maintain accurate billing and documentation in compliance with state and federal regulations. Establish positive interactions with patients, families, and community stakeholders while adhering to infection control, safety, and privacy protocols. This role requires patient lifting of up to 50 pounds, travel between communities, and flexible scheduling including weekends and holidays.
Required Qualifications
- Must be able to lift fifty (50) pounds of dead weight alone
- Must be able to travel between communities during scheduled hours
- Must be able to work varying hours and shifts, including weekends and holidays
- Attends all mandatory meetings, trainings, and assignments as delegated
- Maintain valid state license (where appropriate)
- Graduate of an ACOTE accredited school of occupational therapy
Desired Qualifications
- Prior to beginning patient care for Reliant Rehabilitation, a peer OT may complete a Competency Checklist
