Job Summary
Provide occupational therapy services in a sub-acute/skilled nursing facility setting as a per diem COTA, collaborating with rehab therapists and nursing staff to develop and implement individualized treatment plans, document patient progress, verify status, and ensure continuity of care. Requires NYSED licensure as an OT Assistant, strong computer skills (Outlook/Word/Excel/electronic records), attention to detail, teamwork, and SNF experience. Per diem shifts may include days, evenings, and weekends in a fast-paced environment.
Required Qualifications
- Licensure by the New York State Education Department as an Occupational Therapy Assistant
- Strong computer skills with Microsoft Outlook, Word, Excel, and experience with electronic documentation
- Good attention to detail and timeliness with documentation
- Ability to work well with others in a team setting
- SNF experience required
