Job Summary
Conduct ongoing interdisciplinary assessments, develop and evaluate care plans, and provide physician-ordered services to help patients regain independence in their homes. Teach task-oriented and compensatory techniques to restore physical function and daily living skills while documenting progress and counseling families on treatment goals. Collaborate with the interdisciplinary team to coordinate care and participate in Quality Assessment and Performance Improvement programs. Requires an active South Carolina license, ACOTE-accredited education, one year of home health experience, CPR certification, and a valid driver's license.
Required Qualifications
- Active license for the state of South Carolina
- A graduate of an occupational therapy education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA)
- One (1) year minimum experience in Home Health industry
- CPR certification
- Valid driver's license
- Car insurance
- Excellent communication skills, written and spoken
- Able to perform and prioritize multiple functions or tasks
