Job Summary
Provide skilled occupational therapy treatment in accordance with established plans of care for adult and geriatric patients in a skilled nursing facility. Implement individualized programs focused on functional outcomes, monitor patient response, and communicate progress with a supervising Occupational Therapist. Maintain accurate documentation to support quality care and reimbursement standards while collaborating with interdisciplinary team members. Requires a current COTA license, accredited program graduation, and experience in skilled nursing or post-acute rehabilitation.
Required Qualifications
- Graduate of an accredited Occupational Therapy Assistant program
- Current Occupational Therapy Assistant license (or eligibility for licensure) in applicable state
- Strong communication, organization, and clinical reasoning skills
- Commitment to patient-centered care and professional standards
Desired Qualifications
