Job Summary
Provide skilled occupational therapy services in patient homes according to the Occupational Therapist plan of care. Educate patients and family members on treatment procedures, safety measures, and proper use of equipment such as wheelchairs, braces, and walkers. Assess functional problems, perform prescribed diagnostic tests, and guide patients in therapeutic activities to improve functional status and foster independence. Document all aspects of treatment, assessments, and patient education within specified timeframes, including subsequent visits within 24 hours and start of care within 48 hours. Participate in case conferencing, complete chart audits, and attend mandatory trainings while maintaining an active license. Drive to assigned locations and adhere to on-call requirements.
Required Qualifications
- Occupational Therapist Assistant with a current license in the state of employment
- Minimum of one year of experience as an OTA in an acute setting
- Valid driver's license and auto insurance with your name as a listed driver
- Travel is required for this position
