Job Summary

Conduct comprehensive evaluations of patients' physical, emotional, and social conditions to identify needs and set realistic therapy goals. Develop individualized treatment plans aimed at improving daily living skills and adapt them based on patient progress. Supervise Certified Occupational Therapy Assistants by providing guidance, reviewing implementation, and using telehealth tools for collaboration. Facilitate daily visits with providers to ensure care coordination and serve as a liaison between therapy services and the medical team. Educate patients and families on therapy goals, provide adaptive equipment training, and maintain accurate documentation of all care activities. Ensure all activities comply with federal and state regulations while working collaboratively with regional leadership and the care group.

Required Qualifications

  • Master's or Doctoral degree in Occupational Therapy from an accredited program
  • Current state license to practice as an Occupational Therapist in good standing
  • Minimum of two (2) years of clinical experience, preferably in a home health or outpatient setting
  • Strong skills in occupational therapy treatment techniques and patient care
  • Proficiency in using Telehealth platforms and electronic health records (EHR)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Strong organizational and time management skills, with the ability to multitask and prioritize responsibilities effectively
  • Ability to read and communicate effectively
  • Strong written and verbal skills
  • Basic computer knowledge
  • Ability to manage and demonstrate effective leadership skills
  • Must be able to work flexible hours and travel between offices, facilities, etc
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order
