Job Summary
Develop occupational treatment plans for new and existing members through comprehensive assessments and in-depth reviews of overarching Plans of Care. Document all visits, care activities, and status changes to ensure cohesiveness across departments and interdisciplinary personnel. Carry out periodic assessments to enhance care plans, evaluate home environments to minimize hazards, and provide in-home support as needed. Educate LIFE Center staff and member families to ensure treatment plans are executed consistently. Collaborate with interdisciplinary teams to inform participants of pertinent OT changes related to medical, social, or emotional care, and work with dietary staff to develop adaptive feeding techniques. This role supports Senior LIFE's mission of helping seniors remain at home in the community.
Required Qualifications
- Licensed occupational therapist in Pennsylvania
- Commitment to participating in necessary classes/workshops to retain license
- At least one year of experience as an OT
- Experience specifically working with an elderly population
- Demonstrated clinical competency as an OT
Desired Qualifications
- Additional management work in rehabilitation or a related medical/social field
