Job Summary
Occupational Therapist delivering age-appropriate rehabilitative care in the patient’s home, collaborating with interdisciplinary team to ensure goals are met. Responsibilities include direct patient care from admission through discharge, development and revision of the plan of care, coordination with Clinical Supervisor and team, patient and caregiver education, staff education and professional development, and thorough documentation within regulatory guidelines. Must have graduate-level OT education, BLS certifications, valid Georgia driver’s license, private vehicle insurance, and ≥1 year of home health experience. Strong critical care assessment, leadership, problem-solving, organization, and communication skills are required.

- Graduate of an accredited school of Occupational Therapy
- BLS - Basic Life Support (1.00 Required)
- BLS - Instructor (1.00 Required)
- BLS - Provisional (1.00 Required)
- Occupational Therapist license (2.00 Required)
- Georgia Driver's License (3.00 Required)
- Private Vehicle insurance (4.00 Required)
- Minimum 1 year of Full-time Occupational Therapy experience (Required)
- Home Health experience preferred
