Job Summary
Deliver age-appropriate occupational therapy interventions for home health patients, managing care from admission through discharge while adhering to physician orders. Conduct assessments, perform therapeutic exercises, and provide education to foster patient independence. Collaborate with multidisciplinary teams, clinical supervisors, and physicians to develop and revise care plans, ensuring accurate documentation in the EMR and timely communication of status changes. Participate in preceptor roles for new employees, attend staff meetings, and complete continuing education to maintain regulatory compliance with CMS and Joint Commission guidelines. Manage patient visits and productivity standards while applying critical care assessment skills within the Georgia home health environment.
Required Qualifications
- Graduate of an accredited school of Occupational Therapy
- Basic Life Support
- BLS - Instructor
- BLS - Provisional
- Occupational Therapist
- Georgia Driver's License
- Private Vehicle insurance
- Minimum 1 year of Full-time Occupational Therapy experience
- Strong critical care assessment and rehabilitative skills
- Positive leadership, problem solving skills
- Strong organizational and time management skills
- Strong written and verbal communication skills
- Use of window based computer
