Job Summary
Provide one-on-one Occupational therapy to older adults in private residences or senior living communities, including evaluations, development of individualized plans of care, and delivery of evidence-based treatment to improve mobility, independence, and functional outcomes. Deliver in-home and community-based OT services, with telehealth/virtual care when appropriate to extend access; educate patients, caregivers, and staff to promote safety and aging-in-place; document in FOX’s EMR per regulatory standards; collaborate with PTs, OTs, SLPs, and Fitness Specialists; and maintain autonomy in scheduling and treatment delivery. Role requires driving to patient homes, lifting and assisting with transfers as needed, and wearing PPE when required. Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
- Ability to drive to patient homes and community-based settings
- Ability to meet physical demands including lifting and positioning patients as part of therapy
- Ability to perform evaluations and develop individualized plans of care
- Comfort collaborating with an interdisciplinary team
