Job Summary
Practice independently in the community as part of an interdisciplinary care team. Conduct comprehensive assessments of patients to determine needs, develop treatment plans, and deliver personalized rehabilitation therapy in homes or care facilities. Constantly evaluate evolving patient needs and adjust plans of care accordingly. This role supports patients recovering from surgery, illness, or accidents, enabling them to heal in their communities. VNS Health is a 130-year-old nonprofit serving over 99,000 patients daily across New York State.
Required Qualifications
- Minimum of one-year occupational therapy experience or successful completion of probationary period as a VNS Health Rehab Intern", "Licensure to practice as an Occupational Therapist in New York State", "Valid driver's license
