Occupational Therapist - Travel Contract

Job Summary
Provide patient-centered occupational therapy in home health within roughly a one-hour travel radius, delivering skilled OT services in patients’ homes, collaborating with interdisciplinary teams to support safe, functional outcomes. Responsibilities include assessing functional independence, creating home exercise programs, caregiver education, and documenting outcomes to support continuity of care within an interdisciplinary care plan. Proficiency with EMR (Home Care Home Base) and ability to travel to patient homes are required; mileage reimbursed. Shift schedule is Monday–Friday; travel-focused with no on-call. Required Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy from an accredited program
- Active Michigan OT license required to start the assignment
- Basic Life Support (BLS) certification from AHA or ARC may be required
- Experience with home health documentation and EMR usage; familiarity with Home Care Home Base preferred
- Valid driver’s license and reliable transportation; ability to travel within roughly a one-hour radius of hub cities as referrals require
