Job Summary
Deliver skilled occupational therapy services in patient homes, collaborating with interdisciplinary teams to help individuals regain daily living skills. Conduct assessments, create home exercise programs, and provide caregiver education while documenting outcomes in the Home Care Home Base EMR. Meet productivity expectations of approximately 30 points per week and travel within a one-hour radius of hub cities to support safe, functional outcomes. This assignment supports professional growth in community-based practice for diverse patient populations.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program
- Active Michigan OT license
- Basic Life Support (BLS) certification from AHA or ARC
- Experience with home health documentation and EMR usage
- Valid driver's license
- Reliable transportation
- Ability to travel within roughly a one-hour radius of hub cities
Desired Qualifications
- Prior home health occupational therapy experience
- Familiarity with Home Care Home Base
- Experience conducting assessments, creating home exercise programs, and providing caregiver education in home settings
- Comfort with video/Teams interviews and remote onboarding processes
