Occupational Therapist - Travel Contract

Job Summary
Travel Occupational Therapist delivering patient-centered OT services in home health settings with clinical judgment. Provide skilled OT in home environments, perform functional assessments, develop individualized treatment plans, document progress, and collaborate with an interdisciplinary team to support patient safety and independence. Requires an active Illinois OT license, degree in Occupational Therapy, and EMR experience (Home Care Home Base preferred). Schedule Monday–Friday with a productivity target of 30 visits per week; orientation is 5 days. Travel role includes housing and travel/license reimbursement.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited educational program
- Active Illinois Occupational Therapy license required to start the assignment
- BLS certification from AHA or ARC may be required
- Experience with Home Care Home Base EMR preferred
- Productivity expectation: 30 visits per week for registered therapists
- Orientation: 5 days
- Experience: Home health experience preferred; candidates new to the setting will be considered
