Job Summary
Provide in-home occupational therapy services to patients recovering from injury, illness, or disability, conducting patient-centered evaluations and developing individualized plans of care. Collaborate with clinical teams while overseeing OTAs and utilizing HCHB electronic medical records where applicable. Travel between patient visits across Franklin County and portions of Madison and Delaware Counties, with mileage reimbursement provided between visits. Requires an active Ohio Occupational Therapy license, BLS certification, and a minimum of 32 weekly hours in evening shifts. No on-call or weekend requirements apply.
Required Qualifications
- Bachelor's, Master's, or Doctoral Degree in Occupational Therapy from an accredited program
- Active Ohio Occupational Therapy license
- BLS certification from AHA or ARC
- Home health experience
- Experience providing OT services in patients' homes
- Assessment skills
- Plan of care development skills
- Oversight of OTAs skills
- Ability to travel within assigned coverage areas
- Travel between patient visits
