Job Summary
Provide patient-centered occupational therapy care in a home health setting, coordinating with interdisciplinary teams to support safe discharge, functional recovery, and independence. Document all care in Home Care Home Base EMR and meet productivity expectations of approximately 30 visits per week for registered therapists. Travel within the assigned geographic area, covering an expected daily mileage of roughly 25 miles to reach home visits. Complete a mandatory 5-day orientation before starting the M-F assignment.
Required Qualifications
- Bachelor's, Master's, or Doctoral degree in Occupational Therapy from an accredited program
- Active Illinois state OT license
- BLS certification from AHA or ARC
- Ability to meet productivity expectations of approximately 30 visits per week for registered therapists
- Ability to travel within the assigned geographic area
- Expected mileage varies, approximately 25 miles per day
- Orientation attendance required
- Orientation duration: 5 days
