Occupational Therapist

Occupational Therapist needed to join Fifth Star Care Management's in-house therapy team in West Point, VA. New grads are welcome. Responsibilities include evaluating residents’ physical, cognitive, and psychosocial abilities; developing and implementing individualized treatment plans to promote independence in ADLs, functional mobility, and leisure activities; delivering direct OT services per physician orders; documenting evaluations, treatment plans, progress notes, and discharge summaries; participating in interdisciplinary care planning; educating residents, families, and staff on adaptive equipment and home safety; supervising COTAs to ensure regulatory and professional standards compliance; and adhering to all applicable policies. Requires a degree in OT, Virginia license, NBCOT certification, and strong communication, organizational, and time-management skills.
