Job Summary
Provide home-based occupational therapy services to patients in their homes, administer and interpret functional assessments, participate in development and revision of the Plan of Care, administer therapeutic treatment to improve/restore strength and function, review and adapt retraining programs, document patient status and progress, participate in care coordination and discharge planning, design community reintegration activities, design/train staff and family in retraining activities, supervise OT Assistant and Home Health Aide performance, and coordinate with the interdisciplinary team to support patient independence and quality of life.
Required Qualifications
- Degree from an accredited Occupational Therapy Program
