Evaluate and treat patients in a SNF setting following an individualized plan of care, including developing plans of care, setting measurable functional goals, and re-evaluating progress to revise goals. Provide excellent care and customer service to patients and family members while collaborating with facility staff to ensure high-quality outcomes. Treat patients, answer concerns, and educate them on injuries, progress, and probable outcomes. Complete timely and accurate documentation for evaluations, re-evaluations, discharges, and daily progress notes. Supervise Occupational Therapy Assistants.
