Job Summary
Evaluate and treat a geriatric population across two Assisted Living Facilities and in patients' homes throughout the Rochester area. Perform occupational therapy evaluations, develop individualized plans of care, and provide one-on-one treatments focused on improving independence, safety, upper-extremity function, and activities of daily living. Manage your caseload with full clinical autonomy while completing accurate clinical documentation and communicating with patients, families, physicians, and the clinical team. Coordinate scheduling and logistics with the administrative team. This hybrid role offers flexibility and autonomy for clinicians seeking diverse caseloads in established facilities and community settings.
Required Qualifications
- Active Occupational Therapist license in New York
- Reliable transportation
- Strong clinical skills
