Job Summary
Perform occupational therapy evaluations in patients' homes and develop individualized treatment plans that improve safety, independence, and quality of life. Provide interventions to enhance activities of daily living, mobility, home safety, and fall prevention, while recommending adaptive equipment and home modifications. Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team, and complete timely, accurate documentation in accordance with home health standards. This role offers flexible scheduling, low productivity expectations, and mileage reimbursement for travel between patient visits.
Required Qualifications
- Current Occupational Therapist license in the state of practice
- Graduate of an accredited Occupational Therapy program
- Valid driver's license and reliable transportation
