Job Summary
Occupational Therapist for Home Health in Mt. Vernon (PRN/Employee). Evaluates and treats home health patients with physical, emotional, cognitive and/or psychosocial deficits. Responsibilities include assessing independence in living skills, designing and implementing treatment plans, documenting progress, educating patients and caregivers, planning/discharging from therapy services, and overseeing care provided by designated personnel. Requires OT degree (Bachelor’s, Master’s or Doctorate) and licensure in applicable states (IL, MO, OK, WI); BLS HCP, valid driver’s license, MV/R check; driving for home visits. Required Qualifications
- Bachelor’s, Master’s or Doctorate degree in Occupational Therapy (OT) or related area as required to obtain licensure in state of practice
- State of Work Location: Illinois, Missouri, Oklahoma, Wisconsin
- Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA)
- Occupational Therapist license per state (IDFPR IL, DPR MO, Oklahoma Medical Board, Wisconsin DSPS)
- Driver’s License and driving record (MVR)
- Work Location: Home Health / Mt. Vernon, IL
