Job Summary
Provide ministry-funded, community-based occupational therapy services in clients’ homes and/or through virtual care. Conduct OT assessments using interviews, standardized tools, and informal techniques (home safety and modifications, equipment prescription, fall prevention, mobility/physical functioning, cognition, stroke rehabilitation, and chronic conditions). Develop and implement evidence-based intervention plans in collaboration with clients and caregivers; assign/supervise OT assistants as appropriate; educate clients and caregivers; complete funding applications when required; maintain thorough documentation (assessments, care plans, progress and discharge summaries) aligned with referral source requirements. Registration in good standing with COTO is required. Requires ability to work independently within a community-based model. Proficiency with MS Office and clinical software; valid driver’s license with vehicle access for community travel. This is an independent contractor role with flexible part-time or full-time opportunities and per-visit compensation, including potential annual earnings between $82,000-$102,000; hybrid caseload with option for virtual visits and assessments.
Required Qualifications
- Registered Occupational Therapist
- Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Valid G or G2 driver’s license with access to a reliable vehicle for community travel
- Proficiency with computer applications, including MS Office Suite and proprietary clinical software
- Experience in community practice is an asset, but new graduates are welcome
