Job Summary
Provide ministry-funded, community-based occupational therapy services in clients’ homes and/or through virtual care with a hybrid caseload (option for virtual visits and assessments). Conduct comprehensive OT assessments (home safety/modifications, fall prevention, equipment prescription, mobility, cognition, stroke rehabilitation, chronic conditions) and develop evidence-based intervention plans in collaboration with clients and caregivers. Assign, supervise, and monitor occupational therapy assistants as appropriate; educate clients and caregivers; complete funding applications; maintain documentation (assessment findings, care plans, progress and discharge summaries) aligned with referral sources and COTO standards. Flexible part-time or full-time opportunities with per-visit compensation; independent contractor role; potential annual earnings between $82,000-$102,000. Requires license/certification in OT with Ontario registration, valid driver's license, and ability to work in a community-based setting; remote/hybrid work possible.”,
Required Qualifications
- Registered Occupational Therapist (OT) in Ontario
- Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Valid G or G2 driver's license with access to a reliable vehicle for community travel
- Ability to work independently in a community-based model
- Proficiency with MS Office Suite and clinical software
