Job Summary
Provide ministry-funded, community-based occupational therapy services in clients’ homes and/or through virtual care. Conduct OT assessments using interviews, standardized tools, and informal techniques (home safety/modifications, equipment prescription, fall prevention, mobility, cognition, stroke rehab, chronic conditions). Develop and implement evidence-based intervention plans with clients and caregivers. Assign/supervise/monitor OT assistants as appropriate. Educate clients/caregivers and connect them with relevant community resources. Complete funding applications when required. Maintain accurate documentation (assessments, care plans, progress/discharge summaries, referrals). Independent contractor with flexible part-time or full-time opportunities based on caseload; compensation is per visit with annual earning potential between $82,000-$102,000. Must be in good standing with COTO and able to work independently in a community-based model. New graduates welcome; strong communication, clinical reasoning, and documentation skills required. Proficiency with computer applications. This is a current vacancy.
Required Qualifications
- Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario (COTO)
- Valid G or G2 driver’s license with access to reliable vehicle for community travel
- Proficiency with MS Office Suite and proprietary clinical software
