Occupational Therapist needed for an independent contractor role in Port Perry, ON, delivering ministry-funded community-based OT services to adults, with hybrid options including virtual visits. Responsibilities include conducting OT assessments (home safety/modifications, equipment prescription, fall prevention, mobility, cognition, stroke rehab, chronic conditions), developing and implementing evidence-based intervention plans with clients and caregivers, supervising OT assistants when appropriate, educating clients/caregivers, completing funding applications, and maintaining accurate documentation (assessments, care plans, progress/discharge summaries) aligned with referral sources and COTO standards. Required Qualifications
- Registration in good standing with the College of Occupational Therapists of Ontario (COTO)
- Valid G or G2 driver's license with access to a reliable vehicle for community travel

- Ability to work independently within a community-based model
- Strong communication, documentation, and clinical reasoning skills
