Occupational Therapist

Provide home health occupational therapy services in patients' residences. Conduct initial evaluations, establish plans of treatment with physician input and short/long-term goals, guide patients in therapeutic activities to maximize function, plan and implement therapy programs, instruct patients and families in equipment use, monitor and document patient progress, coordinate with Rehab Program Manager and other staff, plan discharges, and educate other health team personnel. Requires valid PA or NJ driver’s license with acceptable driving record, ability to transport to patient homes, CPR certification, and a Bachelor's degree in Occupational Therapy with state licensure; Master’s degree preferred. Emphasis on family-centered care, documentation accuracy, and adherence to agency policies.
