Job Summary
Occupational Therapist (OT) for in-home and senior living care in Lafayette, IN. Provide one-on-one therapy to older adults in private residences or senior living communities, conducting evaluations, developing plans of care, and delivering evidence-based treatment to improve mobility, independence, and quality of life. Telehealth/virtual care may be incorporated; collaborate with an interdisciplinary team; document in FOX’s EMR; educate patients, caregivers, and staff to promote safety and aging in place. Flexible schedule with options for full-time, part-time, or PRN/FOX Flex engagements; opportunities to work in-home and in senior living settings with potential telehealth components; professional growth supported. Must hold OT licensure (or eligibility), possess CPR certification, and have basic computer/EMR proficiency. Promotion of ongoing education and career advancement within a clinician-driven culture.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Current CPR certification
