Job Summary
Conduct home visits to clients in designated geographic territories within Senior Living facilities to perform thorough evaluations of functional status, cognitive, and motor aspects. Develop individualized intervention plans, assess home environments for hazards, and identify equipment needs to enhance independent living outcomes. Continually revise care plans, participate in multidisciplinary discussions with nursing, and educate patients and families on rehabilitative care. Accurately document all observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current MD Occupational Therapist license, one year of recent experience, and proficiency with electronic medical records. Base pay ranges from $53.00 to $68.00 per point.
Required Qualifications
- A current MD Occupational Therapist license
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills

- Solid computer skills
