Job Summary
Evaluate patients' functional status, including mobility, strength, coordination, and ability to perform daily activities. Assess home environments for safety risks and develop individualized treatment plans with clear, measurable goals. Provide therapeutic interventions to restore function, fabricate splints, and train patients in adaptive devices. Support patients returning to work through functional evaluations and educate families on ongoing care needs. Document patient care accurately within the EMR and collaborate with physicians and interdisciplinary teams. Supervise Certified Occupational Therapy Assistants and oversee home health aide care when therapy is the sole skilled service. Participate in discharge planning and case conferences to ensure seamless transitions.
Required Qualifications
- Graduate of an accredited Occupational Therapy program
- Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
- Current state licensure as an Occupational Therapist
- Valid driver's license
- Reliable transportation
- Auto insurance
Desired Qualifications
- Minimum of one (1) year of clinical experience
- Home health experience
