Job Summary
Conduct one-on-one occupational therapy for older adults in private residences, senior living communities, and memory care settings to improve mobility, independence, and quality of life. Deliver sessions supporting wellness and longevity while documenting effectively within FOX's EMR system. Collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists, and incorporate telehealth or virtual care when appropriate to extend access. Manage your own treatment schedule with autonomy, providing moderate to maximum assistance with patient transfers and driving to client locations as part of the Geriatric House Calls model.
Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
