Job Summary
Conduct one-on-one occupational therapy for older adults in private residences and senior living communities to improve mobility, independence, and quality of life. Deliver sessions supporting wellness and longevity, document progress in FOX's EMR system, and collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Provide telehealth and virtual care when appropriate to extend access and flexibility. Drive to patient homes and facilities as part of the Geriatric House Calls model, assisting with patient transfers and bed mobility. Required Qualifications
- Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain
- Degree from an accredited Occupational Therapy program
- Basic computer and EMR proficiency
- Must be able to meet the physical demands of in-home and community-based care, including: Providing moderate to maximum assistance with patient transfers and bed mobility
- Must be able to drive to patient homes and senior living communities as part of the Geriatric House Calls model
