Occupational Therapist - Rehab- Occupational Therapy

Confer with physicians and care teams to evaluate patients and develop rehabilitation plans that restore strength and functional independence. Design community reintegration activities and recommend adaptive equipment to support clients in daily living skills like dressing and hygiene. Supervise Occupational Therapy Assistants and Home Health Aides, ensuring accurate documentation of care observations and implementation of interim physician orders. Participate in discharge planning and care coordination while reporting patient progress to the rehabilitation staff. Requires minimum six months of occupational therapy experience and a valid driver's license for full-time, 40-hour weekly employment at CenterWell Home Health.
