Job Summary
Occupational Therapist - Home Health delivering rehabilitation in patients' homes. Administer and interpret functional assessments, collaborate with physicians and the care team to develop and revise the Plan of Care, provide therapeutic treatment to improve strength, range of motion and independence, review and expand retraining programs, document patient care and progress, participate in care coordination and discharge planning, design community reintegration activities, train staff and family members, supervise OT Assistant and Home Health Aide performance, and ensure compliance with physician orders and clinical documentation. Requires a valid driver’s license and reliable transportation; travel to patient homes is expected. Role is part of CenterWell Home Health under Humana with full-time schedule.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver's license, auto insurance and reliable transportation.
