Job Summary
Occupational Therapist for CenterWell Home Health delivering rehabilitation and functional-improvement services in patients' homes. Responsibilities include administering/interpreting function tests, collaborating on the Plan of Care, providing therapeutic treatment per physician orders, expanding retraining programs, documenting care, coordinating with the care team and discharge planning, designing community reintegration activities, and training staff and family in retraining programs. Must have a valid driver’s license with reliable transportation and at least six months of OT experience in Home Health. Full-time role, pay per visit with compensation within Humana CenterWell Home Health benefits context.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
