Job Summary
Deliver occupational therapy services to home health patients to improve strength, mobility and independence. Assess function, interpret tests, collaborate on the Plan of Care, provide therapeutic treatment per physician orders, guide retraining and adaptive strategies, document progress, participate in care coordination and discharge planning, design community reintegration and train staff/family for retaining functional gains, and supervise OT staff and aides. Must have a valid driver’s license and access to reliable transportation; experience in Home Health is required. Role focuses on in-home rehabilitation, functional restoration, and patient/family education, with pay-per-visit compensation structure.
Required Qualifications
- Minimum six months of occupational therapy experience
- Home Health experience
- Valid driver's license, auto insurance and reliable transportation
